"You are hereby notified that pursuant to the Act of Congress, approved May 18, 1917, you are called for military service...you will report at the office of this Local Board for physical examination on Aug 15, 1917 at 8 a.m....." Serial No. 2984.
On verso: Printed form explaining the penalities of making false statements. Includes a handwritten note, presumably by John O. Miller: "Wire Board #3, San Antonio c/o of me at my expense, jurisdiction over physical examination. Impossible to come to Dallas."
